Is anyone aware of when unbleached pastry flour is allowed? Is this a later list or a rare occurence for list 3?

I think this is no different
I think this is no different from regular white flour (only ground finer) so whenever that is allowed occasionally

Yes...
There isn't any difference. Just how it is ground out. You can even get spelt pastry flour, or whole wheat pastry flour. I've used it before.
